Anemia management in ESRD: On the horns of a dilemma

Recently, I have had several queries and personal communications regarding a perceived increase in RBC transfusions for patients with end-stage renal disease (ESRD) on dialysis. The literature is replete with articles surrounding the management of anemia in this patient population, particularly involving the use of erythropoietin-stimulating agents (ESAs). The literature is limited, however, when it comes to clear guidelines for transfusion. In fact, transfusion is most often discouraged in this population if, in particular, the patient is awaiting a transplant where alloimmunization must be avoided.

Q. Ms. Gammon, how did you become interested in blood management?
A. In late 2005, the Joint Commission was approached about the possibility of developing a set of performance measures addressing the topic of blood management. The health care community had raised concerns regarding overuse and misuse of blood despite mounting evidence supporting a ‘restrictive’ policy for transfusions. In response, the Joint Commission proceeded in the development of the Blood Management Performance Measures Project in 2008.
Q. What has been your biggest surprise leading this project?
A. I am surprised that every hospital in the U.S. does not focus on patient blood management.
Q. What are two reasons healthcare providers should care about blood management?
A. This is the first time performance measures have been developed to assess blood use, and it is especially important that it is at the patient level. The topic of Patient Blood Management is rising on the national agenda.
